Website form erases info before going to PayPal

Hi. I recently had some PHP files created for a form on my website at www.TheMaineReview.com/submit.htm, for writers to submit writing for a literary journal.

The information DOES forward to my email, but when the visitor clicks on the “Pay Now” button, all of the info disappears, so it looks like it’s erased – and I wouldn’t want to pay if it looked like my info was erased, so I imagine others might not either! I’d like to fix the problem if I can.

NOTE: During the process of creating the PHP, the url was changed from www.TheMaineCoastJournal.com to www.TheMaineReview.com. Would this cause the “hesitation” and disappearance of the info? Or can you suggest anything else that might fix the issue?
