Query form for a travel website

I consider myself as dumbo on php, please if any one can help me for designing a query form for one travel website, in which there is a use of captha validation also.
Is there any link through which i can use the query form as per my requirement.

What kind of query form do you need? Is this a form to send user info by email, or send a query to a database and return results?

As for CAPTCHA solution, I suggest to use reCaptcha.net, this is a free service provided by Google.
