After a quick look through, it appears fine. Are you receiving
cannot connect
or
cannot select DB
(Database related issues)?
Also, you do not need to place variables inside of speech marks when not joining them together with other parts of a string or other data:
mysql_connect("$host", "$username", "$password")or die("cannot connect");
mysql_select_db("$db_name")or die("cannot select DB");
Can be:
mysql_connect($host, $username, $password) or die("cannot connect");
mysql_select_db($db_name) or die("cannot select DB");
