Help... with php form?

Hi

I have offered to help someone out with there booze business… and to save them money instead of ringing the drivers when a client calls reception … they want like a form.

What we want:
a form which the receptionsit will enter the information
eg:
Name:
TeL
Address from
Address to
Time
Date

Then what we want is for the drivers to be able to look on a secure page at the list of jobs and be able mark the job to say they are doing it, then be able to delete the job once they have done it?

So, what is your question? You cannot create HTML form? Check this example on how to save form data to a database. And here is example how you can delete record from a database.
