Contact form, need help

I’ve copied a site template from http://www.css3templates.co.uk. but the concact page in php is not displaying correctly. I really am not good at php, or any scripting at all. So I really need some help from people who understand php.

Where is it messing up? you need a more specific doc type, as most browsers use that to determine how to display the place. is it strict, transitional, etc.
