Cant get php form to work!!!!

I am creating a simple HTML site for a client. He informed me he wanted a form that goes to his email. I found a very simple php form that would work perfect but I cant get it to work! When the form is filled out and you hit Submit it says:

"We’re sorry, but there’s errors found with the form you submitted.These errors appear below.

We are sorry, but there appears to be a problem with the form you submitted.

Please go back and fix these errors."

I have followed the instructions on the html_form_send.php instruction file (although I did add iin the City field but added it where it should have been in the file) and it still gives me that error.

I obviously am not a php coder…CMS design is my forte…I need help with this one.

Your problem is likely right here:

<label for="first_name">What City do you offer Services In? *</label><input type="text" name="first_name" maxlength="100" size="80">

Notice how you are using “first_name” instead of “city” for the input name and label?

That you sooooo much! I misnamed ALL of them…LOL…and I have been tearing my hair out for hours!

Think you can assist with 1 more thing? Now that the form works I want it to redirect to my thank_you.html page but at the bottom of the code page I sent you above it just says “put your HTML code here” so I did and its not working.

Do you know the php trick to after they hit Submit it goes to the thank_you.html page?

After @mail do a header redirect:
