I am an it of our company and i want to put an online registration on our website. We have an Online registration form and a signature card. Everytime a member applies for registration a number of registrant will be recorded like this:

List Of Members Applied:

carmen : 6 (click link para ma print namu ang members nga nag apply)
bulua : 2 (click link para ma print namu ang members nga nag apply)
cogon : 4 (click link para ma print namu ang members nga nag apply)

i have an online membership form which is needed to be uploaded on our website so that every members who wants to be a member need to access our website and click on our online membership form and fill up needed information. once the form is complete they need to click on submit so that every registration will be counted wherever branch he/she wants to be a member for. i will attach the form that needs to be put on our website.

Okay, So, a data table to store the data.
A HTML form for the user to input information.
And processing code to put the data into the database.

On the admin side, a query to count the applicants.
A link to the users app

exactly, do you have a php script that would count how many applicant are there in one branch just to have an idea

You would just do a count in the query. There isn’t a specific script.

What do you have currently?
